Key Findings

Approximately 150 mouse droppings observed on windowsills, walls, inside refrigeration motor housing, and food equipment; approximately 100 rat droppings found in rear storage area. About 20-30 large flies also spotted throughout premises.
Food handlers prepared raw fish and seafood without a certified food service manager on site or valid Chicago food service certificate posted.
Food containers stored directly on floors throughout store and storage areas instead of being elevated at least six inches on shelving.
Three containers of unapproved food items (oxtails, rice, bread dough) and dried king fish with no labels or source proof were found for sale; all were discarded during inspection.

Inspector's Notes:

OBSERVED EVIDENCE OF RODENT INFESTATION ON THE PREMISES. OBSERVED APPROXIMATELY 150 MOUSE DROPPINGS ON WINDOWSILLS, ALONG WALLS, INSIDE REFRIGERATION MOTOR HOUSING, ON FOOD EQUIPMENT AND THROUGHOUT THE STORE. ALSO OBSERVED APPROXIMATELY 100 RAT DROPPINGS IN THE REAR STORAGE AREA UNDER UNSED SHELVES AND THROUGHOUT AREA. MANAGEMENT INSTRUCTED TO CLEAN AND REMOVE ALL RODENT DROPPINGS FROM ALL AFFECTED AREAS. ALSO OBSERVE APPROXIMATELY 20-30 LARGE FLIES THROUGHOUT PREMISES. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO ELIMINATE THE PEST ACTIVITY. PRIORITY #7-38-020(A). CITATION ISSUED.
