OBSERVED 1 LIVE ROACH, 8 SMALL FLIES, AND 2 BIG FLIES ON SITE AT THE TIME OF THE INSPECTION. THE LIVE ROACH WERE OBSERVED IN THE BASEMENT AREA WHERE THE CHEST FREEZER IS STORED, THE SMALL FLIES IN THE BASEMENT AREA AND TOILET ROOM, THE BIG FLIES FLYING AROUND THE FACILITY. INSTRUCTED TO ELIMINATE LIVE ROACH, SMALL AND BIG FLY ACTIVITY, CLEAN AND SANITIZE AFFECTED AREAS AND RECOMMENDED CONTACTING PEST CONTROL FOR A SERVICE.
