• Over 300 mouse droppings discovered on floors and shelves in basement prep and storage area; a dead mouse was found inside a food container.
• Two containers of soup held at unsafe temperatures (73.2°F and 77.5°F) outside of hot holding units; 5 lbs of soup was discarded.
• Open pest control bait blocks left on basement floor instead of in enclosed bait stations, creating contamination risk.
• Mold observed on basement ceiling; kitchen floors around equipment and drains covered in dirt and grease; expired food handler and allergen training certificates on staff.
