Fail

A routine, unannounced visit by a city health inspector.

Inspectors found approximately 100+ mouse droppings throughout the facility, including in food prep areas and equipment.

Standout Inspection

We flagged this inspection for serious public health concerns.

Key Findings

  • Extensive rodent and pest infestation: roughly 100+ mouse droppings found on floors, inside fryer cabinets, storage areas, and equipment throughout first and second floors; live flies observed at hand sink; dead water bugs under dish machine.
  • Food handlers failed basic hygiene practices: dishwasher handled dirty dishes then removed clean dishes without washing hands; employees wore same gloves while handling raw foods, garbage, and ready-to-eat foods; employee drank from unsecured glass in food prep area.
  • Temperature control failures: eggs stored at 63.9°F, sliced ham at 44-46°F, cooked rice at 51-55°F, and raw chicken at 47.3°F (all above the 41°F requirement); two-door cooler running at 50°F held TCS foods; approximately 116 pounds of product discarded.
  • Missing compliance documentation: employees lack current food handler training certifications; food managers missing required allergen training certifications; hand sink on second floor has slow drain and needs repair.

Key findings are derived directly from the inspector's notes from the Chicago Department of Public Health. See the full violation notes below.
