Key Findings

  • Employee touched cash at register, then immediately put on gloves and prepared food (toasting buns, adding toppings) without washing hands in between. Manager had the food discarded.
  • No certified food manager on duty when time-temperature control foods like chicken and rice were being prepared and served. A manager with valid certification arrived later.
  • Approximately 60+ live flies and hundreds of live black larvae (maggots) found crawling on walls, floors, ceiling, and food storage areas throughout the restaurant including prep areas, dining room, and restrooms.
  • No signed employee health policies on file. Door gap at north parking lot entrance allows pest entry. Walk-in cooler door gasket worn and needs replacement.
