• Two hoses were draining directly into an exposed handwashing sink in the front prep area, creating a backflow and contamination hazard.
• Paper towel dispenser was improperly placed at a dump sink in the bar instead of at a proper handwashing station.
• Heavy buildup of dirt, grime, and food debris accumulated under the cook line, rear dish machine, and inside an unused ice bin at the water station.
• Dust and debris had accumulated heavily on the hood and filters above the oven in the rear prep area, and baseboards were peeling away from walls.
