OBSERVED APPROX 10-15 LIVE ROACHES LOCATED UNDER THE SHELVING ABOVE THE THREE COMPARTMENT SINK IN THE REAR DISH WASHING AREA. ALSO OBSERVED APPOX 10-15 LIVE ROACHES UNDER THE LEFT SIDE OF THE THREE COMPARTMENT SINK WHERE THE WATER PIPES GO INTO THE CONCRETE IN THE WALL. ALSO OBSERVED 50 LIVE ROACHES AT THE PREP COUNTER LOCATED BETWEEN THE COLD PREP STATION AND THE HOT HOLD UNIT. INSTRUCTED TO CONTACT PEST CONTROL AND HAVE A SERVICE OF PEST CONTROL COMPLETED BEFORE WE RETURN FOR THE INSPECTION. PRIORITY VIOLATION 7-38-020(A) CITATION ISSUED.
OBSERVED BLACK SUBSTANCE BUILDING UP INSIDE OF THE ICE MACHINE AND A SLIME LIKE SUBSTANCE BUILDING UP IN THE ICE MACHINE AS WELL AS THE MACHINE INSIDE IS BROKEN AND SEPERATING AT THE FRONT. INSTRUCTED TO REPLACE ICE MACHINE AS IT MAY CAUSE POSSIBLE CONTAMINATION.
