LAST PEST SERVICE REPORT DATED 8-28-26 FROM MEN IN BLACK DID NOT TREAT FOR FLY ACTIVITY ONLY ROACH AND RODENT. NO PEST TREATMENT SINCE THAT REPORT. INSTRUCTED TO HAVE THE PEST CONTROL TREAT ESTABLISHMENT FOR FLIES AND ROACHES. INSTRUCTED TO ELIMINATED ALL FLY AND ROACH ACTIVITY. PRIORITY VIOLATION 7-42-090 CITATION ISSUED.
