Key Findings

• Over 200 mouse droppings observed in storage rooms, bar, prep areas, and dishwashing area. Facility must eliminate rodent activity and hire licensed pest control.
• Food handler training certificates provided were expired. Valid certificates required for all food handlers.
• Menu missing required disclosures for undercooked animal-derived foods: no asterisks marking these items, no statement explaining what asterisks mean, and no reminder to consumers about risks.
• Missing splash guards on handwashing sinks in dishwashing area and bar, and rust on cooler shelving units that cannot be properly cleaned.
