• Approximately 5 live roaches observed on walls, floors, and under equipment in food prep and storage areas. Additional pest control required.
• No valid Chicago food service sanitation certificate on file; facility must have certified manager present whenever time-temperature controlled foods are prepared, handled, or served.
• Food stored in coolers and dry storage without proper labeling of contents.
• Uncovered food items found in walk-in freezer, on prep tables, and in bulk containers; milk crates used as shelving instead of proper shelving 6 inches off floor.
