6-501.111 OBSERVED EVIDENCE OF PEST ACTIVITY ON SITE. OBSERVED ABOUT 25 RODENT DROPPINGS ON TOP OF THE HOT WATER HEATER LOCATED IN THE REAR, ABOUT 25 RODENT DROPPINGS ON THE FLOOR AROUND THE HOT WATER HEATER AND UNDER SHELVES, ABOUT 10 RODENT DROPPINGS ON THE FLOOR IN THE TOILET ROOM, ABOUT 10 RODENT DROPPINGS IN THE DRY STORAGE SHED ON THE SHELVES, ABOUT 20 RODENT DROPPINGS UNDER THE 3 COMPARTMENT SINK AND ABOUT 25 RODENT DROPPINGS INSIDE OF THE CLOSET THAT IS LOCATED BY THE PIZZA PREP STATION. ALSO OBSERVED ABOUT 3 LIVE LARGE FLIES FLYING AROUND THE FRONT AND REAR FOOD PREP AREAS. INSTRUCTED TO ELIMINATE PEST ACTIVITY, CLEAN AND SANITIZE AFFECTED AREAS AND RECOMMENDED CONTACTING PEST CONTROL FOR A SERVICE. PRIORITY FOUNDATION. CITATION ISSUED 7-38-020(A).
