• Approximately 40 or more small flies were observed throughout the facility, including on food storage containers, shelves, ceilings, and cup storage boxes.
• The ice container at the front serving line was left uncovered when not in use, risking contamination.
• Food handlers were observed preparing food without hair restraints in the food prep area.
