  • Over 50 rat droppings observed on floors and walls in corridor, rear prep area, and front prep area. Dead roaches also found on floor and equipment in rear prep area.
  • Quarter-inch gaps at bottom of front and rear doors allow pests to enter the facility.
  • Bulk containers in prep area unlabeled, and packaged flan in display cooler missing required labeling with business name, address, and ingredients.
  • Single-service articles not inverted for protection, soiled wiping cloths improperly stored on sinks and prep tables, and bowls used as dispensing utensils instead of long-handled utensils.
