OBSERVED THREE LIVE ROACHES AND APPROXIMATELY 5-8 SMALL FLIES. THE LIVES WERE OBSERVED INSIDE OF THE TWO COLD HOLDING UNIT ACROSS FROM THE THREE COMPARTMENT SINK IN THE REAR OF THE FACILITY. THE SMALL FLY ACTIVITY WAS OBSERVED NEAR THE REAR DISHWASHING/PREP AREA AND FRONT PREP/SERVICE AREA. INSTRUCTED TO ELIMINATE LIVE ROACH AND SMALL FLY ACTIVITY, CLEAN AND SANITIZE AFFECTED AREAS AND RECOMMENDED CONTACTING PEST CONTROL FOR A SERVICE. PRIORITY FOUNDATION VIOLATION 7-38-020(A), CITATION ISSUED
