Fail

A routine, unannounced visit by a city health inspector.

Approximately 200 mouse droppings found throughout storage room where food is kept.

Standout Inspection

We flagged this inspection for serious public health concerns.

Key Findings

  • Approximately 200 mouse droppings discovered throughout hallway storage room along baseboards and on boxed items and shelving where canned foods and disposable dishwares are stored. Pest control service required, and all droppings and affected areas must be sanitized.
  • Hot-held burger patties and pureed foods for special needs students held at dangerously low temperatures (117-122°F) for 45 minutes instead of the required 165°F.
  • Missing paper towels at handwashing sink on service line during inspection (provided by manager during visit).
  • Multiple facility hazards: raw wooden beam under microwave that needs sealing, unused dishwashing machine creating pest risk, large uncovered hole in floor at three-compartment sink, and non-functional lighting in walk-in freezer.

Key findings are derived directly from the inspector's notes from the Chicago Department of Public Health. See the full violation notes below.
