• Ice machine's upper ice shoot contained black and brown slime-like substances on ice-contact surfaces.
• Two of three kitchen handwashing sinks had no paper towels available; one sink was blocked by a hose used for cleaning the deep fryer.
• Approximately 3 pounds of cooked, diced chicken expired after only 6 days in cooler and was discarded.
