Mouse droppings and live roaches found throughout kitchen, including in coolers and near food preparation areas.

Key Findings

  • Mouse droppings discovered in two-door reach-in cooler, behind walk-in cooler, and on containers near dish machine. Approximately 10 live roaches observed on floor in rear prep area, on three-compartment sink, and on wall behind sink.
  • Water backs up between sink basins in the three-compartment sink and handsink, requiring immediate repair.
  • Previous violations from prior inspection remain uncorrected: reach-in cooler shelves dirty, dish machine interior has buildup, multi-use utensils stored improperly, floors and walls in rear prep need cleaning, and hood filters clogged with grease.
  • Microwave ovens in front prep area not clean, and certified manager lacks required allergen training certification on site.

OBSERVED APPROXIMATELY 10 LIVE ROACHES ON SITE IN THE FOLLWING AREAS; ON FLOOR IN REAR PREP AREA NEAR DISH MACHINE, ON THREE COMPARTMENT SINK, ON WALL BEHIND THREE COMPARTMENT SINK.
