OBSERVED ONE RAT DROPPING BEHIND A PREP TABLE IN THE FOOD PREP AREA AND ABOUT 10 RAT DROPPINGS LOCATED ON THE FLOOR IN VARIOUS AREAS UNDER SHELVES LOCATED IN THE BASEMENT STORAGE AREA. OBSERVED ABOUT 10 LIVE COCKROACHES CRAWLING ON THE WALL BEHIND PIPING UNDER THE PREP TABLE THAT IS LOCATED NEXT TO THE SODA MACHINE/ICE MACHINE ON THE 1ST FLOOR FOOD PREP AREA. OBSERVED ABOUT 5 LIVE SMALL FLIES FLYING AROUND IN THE DISH WASHING AREA. INSTRUCTED TO ELIMINATE PEST ACTIVITY, CLEAN AND SANITIZE AFFECTED AREAS AND RECOMMENDED CONTACTING PEST CONTROL FOR A SERVICE. PRIORITY FOUNDATION. CITATION ISSUED 7-38-020(A).
