• Inspector observed one live roach crawling from floor drain to dish machine, several dead roaches on floor, and approximately 30+ live small flies on walls, ceilings, and in prep, dishwashing, and storage areas.
• Dish machine sanitizing utensils at 110°F instead of required 180°F final rinse temperature; machine tagged out of service.
• Bulk food storage containers not labeled; multi-use utensils not inverted on shelves during storage.
• Heavy grease and food debris accumulation on grill tables, fryer cabinets, coolers, and walls; rust on walk-in cooler shelving; worn door gaskets on freezer and cooler doors.
