OBSERVED APPROXIMATELY 15-20 DEAD ADULT AND ADOLESCENT COCKROACHES IN THE FOLLOWING AREAS: BENEATH AND BEHIND COOKING AND FRYING EQUIPMENT IN THE MAIN KITCHEN AREA COOK LINE, BENEATH ICE HOPPER IN THE COFFEE BAR AREA, IN CABINETS BENEATH SALAD BAR AT THE SERVICE LINE AREA. INSTRUCTED TO REMOVE DEAD INSECTS AND CLEAN AND SANITIZE AFFECTED AREA(S) TO REMOVE DECOMPOSING INSECTS AND PREVENT BECOMING AN ATTRACTANT TO OTHER PESTS.
OBSERVED FOOD EMPLOYEE WITH CITY OF CHICAGO CERTIFIED FOOD MANAGER CERTIFICATE ON-SITE WITH EXPIRED ALLERGEN AWARENESS CERTIFICATE AT TIME OF INSPECTION. INSTRUCTED TO PROVIDE ALL FOOD EMPLOYEES WITH CITY OF CHICAGO CERTIFIED FOOD MANAGER CERTIFICATE WITH ADDITIONAL ALLERGEN AWARENESS TRAINING AND MAINTAIN CERTIFICATE(S) ON-SITE AND AVAILABLE FOR INSPECTOR REVIEW AT ALL TIMES.
