OBSERVED EVIDENCE OF RAT ACTIVITY ON SITE. APPROXIMATELY 50-65 RAT DROPPINGS OBSERVED ON SITE ON THE MAIN LEVEL AND BASEMENT FOOD PREP/ DISHWASHING AREAS. THE FOLLOWING AREAS WERE OBSERVED WITH RAT DROPPINGS ON THE MAIN LEVEL: UNDER THE LOW TEMPERATURE DISHWASHING MACHINE EQUIPMENT, UNDER THE COOKING EQUPIMENT. THE FOLLOWING AREAS WERE OBSERVED WITH RAT DROPPINGS IN THE BASEMENT: ON THE SIDE OF THE STAIRCASE LEADING INTO THE BASEMENT, IN THE STORAGE AREA UNDER THE STAIRCASE, UNDER THE SHELVING UNIT WITH BOXES OF CONTAINERS STORED, NEAR THE HOT WATER TANK AND THE GREASE TRAP. INSTRUCTED TO REMOVE RAT DROPPINGS, CLEAN AND SANITIZE AFFECTED AREAS AND RECOMMENDED CONTACTING PEST CONTROL FOR A SERVICE. PRIORITY FOUNDATION VIOLATION 7-38-020(A), CITATION ISSUED
