Roughly 70 mouse droppings discovered under shelves in dry storage, chemical closet, cooking equipment storage, behind coolers, basement, and kitchen entrance. About 15 small flies also spotted in prep areas. Pest control recommended.
Black mold-like substance found on ice machine's food-contact surface. Ice is used for human consumption and must be cleaned and sanitized.
Food handler observed touching ready-to-eat potatoes with bare hands. Staff must use utensils, gloves, or dispensing equipment for ready-to-eat foods.
Dish machine sanitizer reading 0 PPM—inadequate sanitization. Front entrance and basement doors have gaps allowing pests to enter.

FOUND APPROX. 70 MOUSE DROPPINGS ON SITE IN THE FOLLOWING AREAS: UNDER ALL SHELVES IN DRY STORAGE, IN LEFT CORNER OF CHEMICAL STORAGE CLOSET, UNDER ALL COOKING EQUIPMENT STORAGE, ON LEFT SIDE AND BEHIND COOLERS AT FRONT BAR, THROUGHOUT BASEMENT, AT ENTRANCE HALLWAY OF KITCHEN. INSTRUCTED MANAGER TO REMOVE DROPPINGS, CLEAN, SANITIZE AND MAINTAIN SAID AREAS. ALSO NOTED APPROX 15 SMALL FLIES, LOCATED IN REAR PREP NEXT TO EXIT DOOR AND IN KITCHEN PREP AREA. MUST MINIMIZE SMALL FLY ACTIVITY. RECOMMENDED CONTACTING PEST CONTROL. PRIORITY FOUNDATION VIOLATION. 7-38-020(A). CITATION ISSUED.
