Fail

A routine, unannounced visit by a city health inspector.

Inspectors found extensive rodent droppings throughout the facility and live flies in food prep areas.

Standout Inspection

We flagged this inspection for serious public health concerns.

Key Findings

  • About 65 rodent droppings observed in multiple locations including under hot holding units, dry storage, behind soda storage, and in the bakery prep area. Live flies also spotted near the dish machine and juice prep equipment.
  • Facility was setting its own rodent and insect glue traps; inspector ordered only pest control to handle trap placement.
  • Front entrance doors have gaps (1 inch between doors, 1/2 inch underneath) allowing rodent entry; doors must be sealed tight.
  • Large hole in bakery prep area wall near cooking equipment creates pest entry point and must be repaired to be smooth and easily cleanable.

Key findings are derived directly from the inspector's notes from the Chicago Department of Public Health. See the full violation notes below.
