• Extensive mouse droppings observed on floors under shelving, along walls, behind coolers, and around equipment in prep area and utility storage.
• Dish machine not sanitizing food equipment—chemical sanitizer reading was zero PPM instead of required 100 PPM. Machine tagged and cannot be used until repaired and reinspected.
• Handwashing sink blocked by large bulk container lids stored inside it, making the sink inaccessible for employee handwashing.
• Single-service articles not inverted and stored on floor instead of elevated, and food handler training expired for staff.
