OBSERVED OVER 12 LIVE COCKROACHES THROUGHOUT ESTABLISHMENT. LIVE COCKROACHES WERE FOUND AT THE BAR AREA, KITCHEN PREP AREA 1ST FLOOR AND IN PREP AREA IN BASEMENT. MUST ELIMINATE ALL EVIDENCE OF PEST ACTIVITY ON PREMISES. MUST CLEAN AND SANITIZE ALL AFFECTED AREAS. PRIORITY FOUNDATION VIOLATION. 7-38-020(a). CITATION ISSUED.
