  • Quarter-inch gaps at the bottom of front and rear doors allow pests to enter the facility.
  • Soiled wiping cloths stored directly on prep tables instead of in sanitizing solution.
  • Debris accumulating on prep tables, cooking equipment, and coolers; missing wall vent cover and gaps around pipes and baseboards throughout facility.
