  • Previous core violations from April 2026 were not corrected, including missing handwashing signage, unlabeled bulk ingredients, an open front door, a broken cooler door, dirty equipment, and floors needing deep cleaning.
  • No certified food manager was on-site while the facility was open and operating.
  • Approximately 12 flies were observed flying throughout the entire facility, and three sticky traps filled with 15+ dead flies were stored directly above food prep tables.
  • Frozen food was being thawed improperly in a rinse basin without running water, and no food temperature probe thermometer was available.
